WHAT DO I NEED TO DO TO PREPARE FOR MY VISIT WITH ARENA EXPRESS CARE?

Bring a photo ID and insurance card.

Select a preferred pharmacy where you would like your prescriptions to be sent.

Gather any relevant health information or medical records that may help your physician during the visit.

Bring a complete list of current medications, including dosages.

WHAT IS MY OUT OF POCKET COST TO BE SEEN AT ARENA EXPRESS CARE?
Your out-of-pocket cost will depend on your insurance plan. Before your visit, please confirm whether your plan includes a deductible, co-insurance or copay.
